We conduct evaluations of work stations in order to reduce employee injury.  We have been using paper-based checklists on clipboards, then entering the data into spreadsheets, and then sending reports back to employees and supervisors for workstation improvement.

Forms Development Time:

I was able to get my first form out in four hours and have created additional upgrades and modifications.

Installation:

We are now using Pendragon Forms to conduct evaluations of ergonomic conditions.  The data is synchronized to Excel and then sent to Word/Outlook to automatically create and send a report to the employee.
